Enrolled / Registered Nurse @ Home Care
Job Description:
Responsibilities:
- Provide quality nursing care and emotional support to ensure patient safety, comfort, and well-being.
- Assess patients’ conditions, monitor vital signs, and administer medications and treatments accurately.
- Develop and implement individualized care plans in collaboration with the healthcare team.
- Conduct home or hospital visits and manage home care cases.
- Support nursing cases and undertake respite assignments when required.
- Guide care staff in managing medical issues and emergencies.
- Communicate clearly with patients’ families on care updates and progress.
- Coordinate care with internal teams and community partners.
- Ensure compliance with MOH documentation and audit requirements.
- Assist with complaint handling, crisis management, and other duties as assigned
Requirements:
- Minimum Nitec/ Diploma/ Bachelor’s Degree in Nursing.
- Advanced Diploma in Gerontology or Palliative Care is an advantage.
- Must be an Enrolled Nurse or Registered Nurse with the Singapore Nursing Board (SNB) and possess a clean disciplinary record. (Nurses on conditional licenses are not eligible for this role.)
- Minimum 2 years of clinical experience.
- Proficient in Microsoft Office and comfortable using IT tools in daily work.
- Ability to converse in local dialects will be an added advantage.
- Willingness to perform occasional shift work, particularly to support palliative care patients.
