SOCIAL WORKER
Job Responsibilities
- Conduct comprehensive assessments covering functional, environmental, biopsychosocial, financial, employment, housing and educational factors.
- Develop appropriate case plans based on each child’s and family’s circumstances.
- Prepare assessment reports according to the required standards and timelines for review by the Senior Social Worker.
- Interview service users and their families to assess and review their situations.
- Provide relevant information, guidance and counselling support to families.
- Coordinate suitable support services to help children and their families achieve the best possible quality of life.
- Recommend appropriate courses of action based on each case.
- Maintain professional relationships with service users while acting as a guide, supporter, advocate and intermediary.
- Liaise with relevant agencies and make referrals when necessary.
- Participate in multidisciplinary team meetings.
- Maintain accurate case records and update information promptly.
- Prepare reports for governing agencies and government means-testing applications.
- Work collaboratively with internal and external professionals to support children’s developmental needs.
