Healthcare Coordinator

Job Responsibilities:
Patient Outreach & Enrolment
- Contact patients and encourage them to enrol in the programme.
- Follow up with patients who do not respond and guide them on how to use the chatbot to submit their responses.
- Check patient enrolment status and ensure eligible patients are correctly recorded in the dashboard.
Patient Monitoring & Coordination
- Monitor the daily dashboard and review chatbot responses to ensure they are directed to the clinical team promptly when needed.
- Follow established procedures to identify cases that require further attention or escalation.
- Coordinate appointments, referrals, handovers and communication between patients and healthcare teams.
Data Management & Reporting
- Update and maintain accurate patient records and databases.
- Record patient interactions, follow-ups, outcomes and escalations in the relevant systems.
- Collect and check data on enrolment, response rates, outreach attempts, escalations and patient outcomes.
- Analyse data to identify trends, gaps in care, repeated non-responses and areas for improvement.
- Prepare regular reports and updates, ensuring information is accurate and up to date.
Audit & Service Improvement
- Support audits, service reviews and initiatives to improve work processes and patient engagement.
Job Requirement(s):
- Minimum Diploma or equivalent experience in healthcare administration or related discipline
