Job Responsibilities:
Patient Outreach & Enrolment
- Reach out to patients and assist them with joining the programme.
- Follow up with patients who have not replied and explain how to respond through the chatbot.
- Track enrolment progress and ensure eligible patients are correctly listed in the system.
Patient Monitoring & Coordination
- Check the daily dashboard and ensure chatbot replies are sent to the clinical team when necessary.
- Identify cases that need further follow-up and refer them to the appropriate team.
- Arrange appointments, referrals and handovers while keeping patients and healthcare staff informed.
Data Management & Reporting
- Maintain and update patient information in the relevant systems.
- Record patient contacts, follow-ups, results and cases requiring further action.
- Gather and verify data on enrolment, patient responses, outreach efforts and outcomes.
- Review data to identify common issues, missed follow-ups and areas for improvement.
- Prepare regular reports and ensure all information is correct and up to date.
Audit & Service Improvement
- Assist with audits and service reviews to improve work processes and patient engagement.
Job Requirement(s):
- Minimum Diploma or equivalent experience in healthcare administration or related discipline
