Job Responsibilities:
Patient Engagement & Enrolment
- Contact patients to introduce the programme and guide them through the enrolment process.
- Follow up with patients who have not responded and help them submit their replies through the chatbot.
- Monitor enrolment records and ensure eligible patients are registered correctly in the system.
Patient Follow-up & Coordination
- Review the daily dashboard and ensure patient responses are shared with the clinical team promptly when needed.
- Identify cases that require further attention and refer them to the relevant team for follow-up.
- Coordinate appointments, referrals and case handovers while keeping patients and healthcare teams updated.
Records & Data Reporting
- Keep patient details and programme records accurate and up to date.
- Document patient interactions, follow-up actions, outcomes and cases referred for further attention.
- Collect and check data on enrolment, response rates, outreach activities and patient outcomes.
- Review data to spot recurring issues, missed follow-ups and opportunities to improve services.
- Prepare regular reports and ensure all information is complete and accurate.
Audit & Process Improvement
- Support audits, programme reviews and improvement efforts to enhance work processes and patient engagement.
Job Requirement(s):
- Minimum Diploma or equivalent experience in healthcare administration or related discipline
