Job purpose
- To provide optimal level of care in accordance to established policies and procedures.
- To assist in activities and programs promoting healthy aging for clients.
- Guide Healthcare Assistant on nursing related care to ensure optimal delivery of required care to the clients.

44 hrs per week, shift

Duties and responsibilities
- Comply with approved Nursing SOPs
- Work with multidisciplinary team to analyze and evaluate nursing care plan and ensure care plan is implemented by team.
- Identify symptoms of clients related to illness or decline in functional/cognitive abilities and advocate early attention.
- Mentor and guide HCAs in performing their functional duties
- Assists the EN/RN in ensuring patients take their medications.
- Assist client to manage common age-related conditions such as incontinence and loss of mobility.
- Assist Enrolled Nurse/ Registered Nurse as when required.
- Other additional operational duties as assigned by Centre Manager.
Qualifications & Skills Required
- Minimum GCE ‘N’ Level
- Attained Training in either WSQ Higher Certificate/Advanced Certificate in Healthcare Support (Nursing Care); or Institute of Technical (ITE) Approved Training Centre (ATC) Healthcare (Home Care) program.
- Experience working in an elderly setting is an advantage
- Compassion, critical thinking, and organizational skills
