Health Coach (1 Year Contract) #HBY
We are hiring under Recruit Express headcount and you will be deployed to:
Location: North / Northeast
Salary: $2500 - $3300
Duration: 1 Year
Working Hours: 5 Days / Office Hours
Job Responsibilities
- Conduct health and lifestyle assessments for residents
- Work with residents to set achievable health and wellness goals
- Develop personalised health coaching plans
- Guide residents on healthy lifestyle and behaviour changes
- Support residents with chronic disease self-management
- Monitor and document residents' progress and health outcomes
- Work closely with the Community Health Team to coordinate care
Requirements
- Minimum Diploma / Advanced Diploma in a healthcare-related field
- Good communication and interpersonal skills
- Able to work independently and within a team
- Good organisational and coordination skills
- Proficient in Microsoft Office
