💼 Responsibilities
• Conduct patient outreach and support programme enrolment
• Monitor daily dashboards and patient responses via chatbot
• Route and escalate cases to clinical teams based on established protocols
• Follow up with non-responsive patients and guide them on chatbot usage
• Verify patient eligibility, enrolment status and dashboard records
• Maintain accurate and updated patient databases
• Coordinate appointments, referrals and handovers between patients and clinical teams
• Document patient interactions, outcomes and escalations across IT systems
• Collate and validate programme and patient data
• Analyse trends, care gaps and non-response patterns to improve patient engagement
• Prepare reports and support audits, service evaluation and improvement initiatives

📝 Requirements
• Minimum Diploma or equivalent experience in Healthcare Administration, Care Coordination, Nursing Support, Population Health or related discipline
• Patient-facing healthcare, telehealth or chronic disease management experience advantageous
• Familiarity with electronic medical records and multidisciplinary healthcare environments beneficial
• Good patient communication skills via telephone and digital channels
• Able to follow protocols and escalate patient concerns appropriately
• Comfortable working with spreadsheets, dashboards and healthcare data
• Confident using multiple IT systems and applications
• Organised, detail-oriented and able to manage multiple patient follow-ups
• Able to work independently and collaboratively within a healthcare team
